Once you have selected/confirmed a date and time for mediation,
please fax 727.462.0196, e-mail (cnc@castagnamediation.com) or call 727.446.4221
with the following information:

1. Short style of the case;

2. Names, addresses, phone and fax numbers and e-mail addresses of all
individuals who need to be notified of the mediation;

3. Number of people who will be in attendance (so appropriate conference room
arrangements can be made);

4. Desired location of mediation (I can generally provide conference space in
Clearwater but often travel to whatever location is most convenient for all involved);

5. Any security concerns so that appropriate arrangements can be made at a secure
facility.
